First of all clarify, are you asking from employer point of view or emp[loyee point of view. Some basic requirement are as follows The company must have more than 20 employees and registration with ESIC thereafter. The employees gross salary should not ecxeed 15000 permonth After joining a firm, the employees need to fill adeclaration form and give family photograph and submit to the employer. The forms are then signed and stamped by company and sent to ESI office through concerned person or consultant. The temporary card is given within a weeks time by esic office to concerned person or consultant. The permanent card is sent to your address mentioned in the declaration form along with the ESI stamp on the photograph provided by you. This permanent card is one time and can be used with every / any emploer throu transfer of employer. The Workmen's Compensation Act, 1923, has been amended by Act 45 of 2009, whereby the name has been changed from Workmen's Compensation Act, 1923 to Employee's Compensation Act, 1923.
